CREDIT CARD AUTHORIZATION FORMS

Dear Valued Customer: All sportsbook charges or debit card depositors must receive payments on betting wins via other payment methods. Please print, and complete the credit card authorization form as you have been instructed to by your Customer Service Representative.

Once you have completed the form(s), please fax it, or scan and email it back to us along with a photo copy of your identification and the front and back of your credit card.

Thank you for your cooperation. Your business is greatly appreciated. No withdrawals will be processed without first meeting the requisite 15 days clearance on all credit card and debit card deposits.

Please note: In order that we may best facilitate our customer’s needs, we sometimes add new payment methods that we think might serve well and discontinue those that do not.
